Forms for 2020-21 School Year - Parent Action Needed

Required and Optional Return-to-School Forms

As we start the year virtually, we are requesting that these beginning-of-the-year forms be completed virtually, if possible.  If you are new to Dogwood, you already completed most of these forms when you registered your child. 

Required Forms For All Students

Emergency Care Form (via WeCare)

All students must have an accurate emergency care information form each school year. 

We will use the email address you provide to communicate all emergency and essential information to parents. Also, please make sure your cell phone is listed in the "cell telephone" fields of the emergency care form. We will use the cell number to text parents with emergency and essential information.

Parent/legal guardians are required to update this information through WeCare via their Student Information System (SIS) ParentVue account. 

(Please note that WeCare@School is a part of FCPS 24-7 Learning : Parent View.  In order to log into Parent View it is necessary to have an active SIS Parent Account.  Once the SIS Parent Account is created the same log in credentials will also work for FCPS 24-7 Learning : Parent View. If you need assistance creating a SIS Parent Account, please contact Pam Taylor at @email or 703-262-3100.)

Health Information Form

If you do not need to change your child's health information form, please sign the form and return it. If your child's health information has changed, please indicate the updated information on the health information form. All forms should be emailed to Pam Taylor at @email. 

Student Rights & Responsibilities

Please review the Student Rights and Responsibilities and related resources at www.fcps.edu/srr.  After reviewing the information, fill out the SR&R Parent Acknowledgement Form no later than October 20, 2020.  The form must be filled out one time per student.

Parent Consent for Children to Use Digital Resources

We need parent consent to use many of our excellent online tools and websites when we teach your children. You may look through this list of resources before signing the form.
